This form is for patients, caregivers and patient advocates to submit a self-nomination to participate on the 2026 PQA Patient Advisory Council.
The PQA Patient Advisory Council (Council) advises PQA on what matters most to patients, their families and caregivers as it relates to safe, effective and appropriate medication use. The Council also provides input on strategies to meaningfully incorporate patient perspectives throughout PQA’s work to improve the quality of medication use and patient outcomes. This is a new Council that will meet for the first time in the first quarter of 2026.
Patients, caregivers, family members and advocates have a critical role to play in high-quality medication use. They provide unique and valuable insights about what is important and meaningful in medication use quality, which supports the development of appropriate and useful quality measures, as well as related education, research and tools that support informed health care decisions and better medication use outcomes.
The Council’s work supports PQA’s mission and enhances the organization’s work to advance quality medication use for a person-centered and value-based health care system.
Nominations are due by Friday, December 12, 2025.
